The reason I ask is because there was a witness to the accident that called in to 911, which he put the name and phone number of the witness in the report and what he concluded in the report and what the witness told me are different. The insurance company of the person who caused accident is basing there decision on the officer's opinion and not on the witnesses statement.
there is not a form. if you do not agree with the report you will need to talk to the reporting officer. keep in mind the accident report is a civil document and the information in the report is the officer's opinion. they will only amend the report if there is a mistake of fact.
