It's for all ages, i believe the only requirement is at least 1 child under 18 with an adult so it keeps people without kids from going without someone with 1. If that makes sense.
If you have a WIC or EBT card, admission is $3 per person. You just have to show the card when paying.

Yes you pay an entrance fee. However the yearly is a much better bargain. Your 13 year old is going to like the 2nd & 3rd floors more.

Yes. This is for all ages. Everyone has to pay to get in. Even parents who are just going in to observe their children. Ages 13+ is a great time to introduce this museum. Kids younger than 13 will enjoy it as well. All mine are under 5 and had a great time!
We bought a year family membership. All ages allowed. Great family environment. Reminds me of OMSI in Oregon. Lots of learning activities. Enjoy!

Admission is a bit pricey, bit worth it. For the cost, be prepared to spend a few hours there to get ypur moneys worth. Kids 13 and up will enjoy it.
