You'll have to call your primary care provider.
You always have the legal right to your own medical records. If needed get a "medical records transfer order" sign it and put your own address. They are obligated to send it to u. If they refuse ask for a patient advocate. If they still refuse get a non offiliated patient advocate. This is a legal right.
There was a website included with my billing, gave me information for records.
