Two forms of ID... It can be your ID/license and your birth certificate... If Im not mistaken the gentlemen at the office said just my ID/license would have sufficed.
ID and birth certificate or something that has a proof of birth and social security number such as a document from your doctor and it has to be current within 30-60 days
