I have a durable power of attorney for my sister. We cannot find her Social Security card. And she is physically unable to go to an office. How do I go about communicating and submitting my power of attorney on her behalf

She does not have to go to the office. She can fill out an SS5, as long as she signs and dates it at the bottom, someone else can bring it to the office for her. They also need to bring her valid driver's license or US passport. We will not keep the documents, but we do need to see an original idea with the application. We will then process the application and the card will arrive in the mail between 3 and 6 days later. You can Google form SS5 and printed off any computer.
