I believe it's $99 per tooth. And yes, you pay for everything up front.

I don't know. I had all of my teeth pulled from the beginning. The implants i had were for the lower dentures. They put two on each side, in my jaw. They will eventually hold my bottom dentures in. I won't be able to use them until the end of May. I paid for everything up front.
