Anne's answer entirely correct. I believe they have three levels of pricing. Extractions, etc... are, of course, extra. I have used bargain to top level and have had excellent results with all.

It depends. I got the "Ultimate" package. It was $2,000 + and is guaranteed for 20 years. They have other packages. Check their website.
