The form your physician gave you is part of the items you submit to the state and then the state determines your eligibility for the medical cannabis program. The certification from your doctor does not give you access to medical products. You have 90 days from the date your doctor signed your form to submit it and the rest of the materials to the State, and they are the ones who allow access to medical products. If you come in with only a physician form, you will not be allowed access to medical products. You must have the temporary certification from the State.

The card comes from IDPH, they will issue a temporary after you complete your application. I am unaware that anything else is acceptable.....
