You would need an adult because they ask for a guardian/parent's drivers license information. Hope this helps.
It depends how old you are. I didn't have to give any information for my son, but he's 16. Just had to sign a waiver.
The adult that is responsible for you during your visit can complete the release form. Your mother's conta t information can be provided at the same time. If you return for another visit with your Mom, or a different adult, they will. Red to complete the way we again, until your parent completes it. Then it will stay on file and you will not. Red to compete the waiver ever again, even if you go with a new adult later.
