I have owned a car since 1996. Title I have, MV-999, says one lien recorded. I haven't paid on the car in 20 years, paid and clear. . Do I need a new title?

You can actually get a letter from the original lien holder stating that it has been paid completely and attach that to your title.

Yes my grandma went through this and she had to get a new title
