Two proofs of address, Application, your sss card and birth certificate. If under 18 years of age a parent or legal guardian with guardian papers need to come in to sign for the minor in front of the examiner. Also if under 18, the parent DL or ID will take place of the proofs of address it the address is the same as where the person resides.
