If your missing ID Card isn't expired. A Biryh Certificate or a Social Security Card can work. A Hospital Birth record will not however. Only a State issued Birth Certificate or a Social Securiry Card. If your ID Card is expired over 6 months, Birth Certificate, Social Security Card, Proof of Ohio Address. Like a Utility Bill, a 1099, a Paystub with your address. A list is available and they'll tell you if you call.
No...the law strictly indicates you must have a legal state issued birth certificate....S.S.Card...Or hospital birth records.. Even a combination of would be acceptable.
