Go to the state department website and fill out the paperwork. You can print it off and take it with you to the post office. The price list is on the website.
Hi, if you haven't gotten her passport yet, it's $100 and you need both parents unless Dad is not on the birth certificate. You need her birth certificate, her SSN, and both parents ID's. You'll need to have a personal check, money order or cashier's check for the payment. There is also a $35 acceptance fee and if you need a photo, it's $10, these fees can be paid by credit, debit or cash.
