I would reach out to the corporate hotline they will make it right I'm sure and call the district manager who will then call the store manager.
Fyi we did go to manager is the one that informed us there was no salad,we were eating our second bowl when the kids entree came

Never had a problem like that. We always get salad and bread together, my daughter eats salad, lots of it. I let her have one breadstick. She just started eating off the adult menu recently, but we always get our meals together, always have unless there is something wrong and we have to send it back. Then we just keep eating salad. We usually take half of our entrees home
