Insurance company guidelines do require a minimum BMI of 35 with at least 1 co-morbid condition or a BMI of 40+ to qualify for surgery. We would love to answer any other questions you may have. There are a lot of major factors that will determine if your a good candidate for WLS. That is something you would need to talk to your dr about.
