I am a (17 yr old) teenager and would like to donate, I have my ID and SS however I don’t have a proof of residence. Would it be possible to use an affidavit of residence signed by a notary as proof of residence?

You have to be 18 years old anyways. You can't donate yet

Send your self a letter and don't open it. I don't think an affidavit will be ok
