You need an appointment.
I've always made an appointment but I've definitely seen a couple of people come in while I was waiting and say they don't have an appointment but wanted to donate. I think it's mainly because it wasn't their very first appointment so they didn't need a physical and it wasn't very busy, so they didn't seem to make a big fuss about it.
