When I went to see Borgore I didn't see anybody in the crowd with a camera besides their phone but at a few other events I have seen some people with smaller handheld devices.
The show and group determine that but on safe side nothing to huge
Depends on the show, of the artist has copyright issues then no, but a smaller show it's fine
Went to The Suicide Girls Burlesque Show in 2017. They didnt mind phones, so I dont think a digital camera would be a big deal. I have a ton of shots from that show!
Telephones yes, not sure about cameras.
