If it's a marriage certificate make sure it's the certified copy. Everyone gets a moment copy that's made for display, but they will only take the certified copy you get from the recorders office after the marriage. Some people get it sent in the mail others have to go down and purchase it from them. If it's a court ordered name change just make sure it's an original and not a photocopy. :)
If you changed it through the courts you take the court paperwork in to prove your name change, if it's because you got married you take your marriage certificate in
