In the state Illinois you have to be 18 years old to get a tattoo, and a parent can not sign any form that says it's ok, there is no such thing at any tattoo shop in Illinois that I know of. If a shop says so they are not telling you the truth, and I would be safe to say it is not a reputable shop. They would be breaking the law if they did any tattoos under the age of 18.
