When I signed my mom up they gave us a paper with the case workers extension. Can't say enough about how helpful everyone there was
The front desk people. They are super helpful and nice. You could probably just call
Call or visit, give them your name at the front desk/by phone and tell them you forgot who your case worker is, and that you need to be transferred/see your worker for ect.. And they will tell you who your worker is. Hope this helps😊
