I believe 18. You must fill out LONG form. About previous pets, who will be vet. I take my cat to PAWS for checkups, shots, etc. It's much cheaper, they provide great service. Friday & Sat from 9-4 are walk-in days. No appt needed. On rest of days you must have appt. Can't remember if they're open Sun.
