Yes it's true first you need to close your case, then take you paper to 218 rt. 17 n and open your case but my advise is close it after you get your benifits so you wont be without your benifits

Yes your services are only good in the county in which you reside. Being that you reside in a different county you don't have reapply as if it's the first time. You can ask for a transfer.
