Okay... All states are the same.. if you would have put in the request for name change during the divorce.. the time would have been shorter. But this will take longer.... #1 Go to your county clerk office and pick up a form called "Name change request" (remember) fill out as much info as possible & provide what ever documents required. Some state requires the divorce decree to be notarized- before filling. #2 The fee for the filling is between $20-$30 dollars. #3 You might want to inform all people you do business with that- you're changing your name. #4 Next the court will inform you- of the hearing date. (Not all states require you to attend the name change hearing). But if you receive a summoned- you must attend. Bring birth certificate (with maiden name), and final decree. #4 the Judge Approval: once all the documents & paperwork are verified, the judge should approve your request and provide you with and "official petition approval document"
