Besides the form signed by the doctor, you need current ID or license from CT DMV. You can make appointment or mail it in which will take longer.
Print out the H13B form and have the bottom portion filled out by your doctor prior to scheduling your appointment to come in to the dmv. Otherwise you will leave with nothing.
First you have to be handicap, second schedule an appointment at the dmv and fill out form H-13B if your applying for a handicap plate. If you already have a valid permanent parking permit, no further medical documentation to provide proof of disability will be required. You will, however, be required to complete section A of the Permanent permit form
