You will need to bring proof of ID for yourself and ID for those over 17 in your household, anyone younger please bring Birth Certificate and Medicaid card. Also bring proof of address which is a lease or con Edison bill. We are open Tuesday - Saturday 10:00 - 2 for registration.
Just go there Thursday or Friday 10am to 4pm bring with u I.D a bill with ur address good luck 👍
