The fee is $30.00 yes you have to fill out divorce form. Show your I.D , the form asks for the husband's birthdate. It doesn't take long you get the certificate right there.
Go into the office, pick up a form for a records request of divorce certificate/paperwork (the employees are usually pretty friendly so ask them if you have questions) fill out the form, and turn it into an employee there. There's a fee but I don't remember what it is. The form can also be done online at their website but I don't remember how to get to the correct page. Hope this helps.
