If you're listed on the record, just need to bring your ID, method of payment ($30) & a request form. The request forms are available the lobby or online. Do keep in mind they only have records/certificates for births that happen in Alaska.
$30.00 for the first copy. Additional copies $25.00 each for the same record ordered at the same time. Heirloom certificate is $55.00 and extra copies $50.00 for the same copy ordered at the same time. More fees are required if they are to be used in a foreign country. This is called Apostille and is $12.00 for the first record and $2.00 each per additional copies.
Time of birth is collected by HAVRS in the birth record. It would be located on the certified copy of a birth certificate but not on the heirloom versions. There are cases where this information is missing in our records, such as when it was not entered by the hospital. We also have some gaps in the electronic version of older records that were converted into our current system. We suggest ordering a long form birth certificate, which would be a copy of the original on file and is most likely to list your time of birth. These are not specifically listed on the order form but can be requested and cost the same as a regular “short-form” certified copy.

The fee is $30.00 yes you have to fill out divorce form. Show your I.D , the form asks for the husband's birthdate. It doesn't take long you get the certificate right there.

Yes. The 1st copy is $30 & copies after that are $25 (I'm almost 100% positive on the price). You can go to their website, print & fill out the request form and then go turn it in, or you can go to the office and you should find the request form on the wall to the right of the waiting line. Best thing is that there's usually not much of a line!

11-14 Business days. You don't have to mail it. After marriage bring the sworn certificate signed by you, your spouse, minister/commissioner & two witnesses 18 and up. They'll then process it with $40 in paid fees. Print it and give it to you in person.
The total process is done there. You'll just need a commissioner/minister.
