I have surgery scheduled for 6:45am according to the voice over the phone. The letter in the mail said I must report 90 minutes before the surgery. Do I seriouly need to arrive and register at 5:15am - or is 6:45am the register time?

You need to arrive 90 minutes before your scheduled surgery time. They need to get you checked in and prepped.

You need to go in at 515 in the morning they have to prep you for the operation. At 645 the operation starts.
