Prof of residence, income,family size. Basically they help w/food, clothes, sometimes utilities, prescriptions, glasses and gave a free community lunch daily
You need your social security cards for everyone in home you're claiming for and proof of address, id and sometimes they want proof of your wages for employment.
Im not to sure,but obviously your id,bank statements,and proof of address for you and your children.i believe this is all you need.i hope this helps you.
