I mailed in a form for a last name change. I was married in June of this year. I sent in all the copies necessary, including a check. I have not received the new license. I haven't had a phone call or an email. I'm checking on the status.

If you are a resident they will put the name the same as it is on your green card, to change your last name on the license you have to upgrade it on the green card, if you are a citizen I guess you have to book an appointment and go personally to the DMV
