You should first try to apply online for a duplicate here -- https://www.ri.gov/DMV/duplicate_license If the system does not allow you to do that, you will have to make an appointment at one of our offices. Appointments are made online at www.dmv.ri.gov Look for the blue reservation box on the home page. Make appointment and then fill out proper paperwork make sure you have to forms of ID and proof of address
