Hello I’ve been trying to contact the office via phone with no answer. I need to change my name ,they don’t accept walk in and I can’t even schedule an online appointment What do I do pleas?

If you go to the local office, they will give you a form and an cover in which you need to place the filled in form and all originals that support your request.
