You can use the Disney app to set up fast passes or they have kiosks around the park, where you can go and set up a fast pass. There is usually someone around to assist you, if you need help. Also, if you forget to scan a picture, they keep the photos them and you can retrieve them based on the date and time.
If you booked your trip with wdw in advanced then you can book fast passes up to six months before your trip. Like wise with meals, you can book them up to 12 weeks before the event. You can only pre book three fast passes a day, and you must use all three up before registering new passes. You can get more fast passes via the app ( search your app store ) or via a fast lass qiosk.
You can download the My Disney Expierience App on the App Store or Google Play. It is the official Disney world planning app. You can choose which rides you would like a Fastpass on and also reserve dining plus check the rides' wait times.
They are available up to 30 days from your visit. The quicker you get your tickets ahead of time and create an account the better your selection will be.

You get a fast pass by buying tickets or select season passes. You can do this by calling a Disney travel agent or at their website online. You can also purchase one at the park for the day if wanted.
You will not get them for popular rides unless you book days in advance for magic Kingdom....the popular ones anyways
