If it has not been too long and your old license has the gold star in the upper right hand corner, that is all you need. Otherwise yes two government documents will be needed, like ss card, birth cert., marriage license, valid passport. Then two verification of address like current elect bill, property tax, bill electric bill in your name, or car insurance bill, bank statement etc.
As I remember, birth cert, marriage cert, Bill showing your address, passport if you have one and bring the outdated drivers license
