You have to fill out the application for a replacement it's the same one as getting a license then bring the right paperwork you might even be able to do it online because of covid
If no change in your address you can do online. If a change in address you have to go in person with ID Proof and do at closest DMV Office.Call DMV in Concord and they will have someone make appt for you
