Call the store to provide some information, and the store associates can look up your insurance plan. LC works with Eyemed, VSP, United, Cigna, Metlife, and MES. But, since plans can go by many different names, it's best to have them check for you. Tips: Get the insurance plan checked prior to your eye exam appointment. Be prepared to provide some information, including the name, birthdate, and last four numbers of the policyholder's SSN. In a few cases, the associates cannot look anything up without that info! Waiting until you get to your appointment to have the insurance checked may hold up your appointment - if you would prefer to wait until day of, arrive 15-20 minutes early.
