Confirming what is mentiined above...need to sign a release at one of the facilities.
You'll have to do a medical release form at either location. (If you call they will direct you to the records department if you have additional questions) It didn't take long for my request to be processed but my father had only been there a few times so there wasn't much to obtain.
Have to go in person and fill a form to request them
