The Monorail is fairly close to the Disneyland Hotel and you can ride directly in to TomorrowLand on the Monorail. The exit is directly between Autopia and Finding Nemo Submarine rides. The Grand California has an entrance directly in to California Adventure. Paradise Pier is across the street from the Grand California so you'll have to cross the street or enter from Downtown Disney. All 3 resort hotels are within walking distance, so you really don't need transportation from any of the resort hotels.
There are 3 Disney hotels at Disneyland and all are within walking distance to both parks: Disneyland and Disney's California Adventure. The trams and shuttles are for guests who drive to the parks and use either the huge parking garage or the surface lots nearby. Many non-Disney hotels close by also have a shuttle to the parks, if they're not already so close you could walk.
There is also a tram from the hotels to to downtown disney and the park.
There are lots of shuttles,buses in and out of the park all the time...if a hotel is close to disney they have a shuttle....

The Shuttles that run around Anaheim are called the ART (Anaheim Regional Transit). You will need to purchase a pass to ride them. If your are staying in Anaheim, They take you from near your hotel right to the front gates of the Resort. The drivers are tourist friendly and well acquainted with strollers and children. If you are riding with a large group,(like a school group) you can call them and let them know you are ready at the stop and they will make sure they send a bus big enough to pick you up.

Yes depending On Hotel You Stay At . I usually stay at the Hilton and it stop right on front and does two stops only then to Disney no more then 5 to eight mins tops but yes to answer your question almost all hotel have a bus that go to Disney
