I need to have state tax taken out of my social security check. Do I need to make an appoitment for it or can I request it on line? If i need an appoitment what is the phone # or website/

Went on line to ss site printed form out and mailed it in. Easy and quick. They sent me response back that they did it and what I will now get.

You don't necessarily need an appointment. You can go and request it at the social security office.
