You will need to go to the BMV and will want to bring your identification, and paperwork with you. But you have already passed the tests so as long as it isn't expired then you can ask for a replacement, I think it is around $5-$10
You will have to go there and pay for a replacement. Bring all of the proper identification (e.g. the same paperwork you brought when you first got your permit.) You may also need to retake the written exam.
