It’s San Diego. I was in shorts and a Sweater and didn’t feel under dressed at all.
You will notice majored people are wearing pants and a button down. I personally wear a button down short sleeve vintage style shirt and jeans. I'd stay away from shorts and open toe shoes for men.
It varies from location to location. But just look clean and they wont turn you down. My husband wears dark jeans and a button down. I usually wear leggings and a button down. No flip-flops.
No. I thought it was when i first went but some people were really relaxed attire wise. I always dress decent though.

Not that I know of. It is a very nice restaurant and most people are dressed nice. I do not think they will turn you down based on your clothing.

Nice dress casual or above. I wear dress pants and blouse. Men wear nice jeans and dress shirt or suit pant w/no tie with or without jacket.
They won't refuse you, but a collared shirt is curtious and no holes in your pants for men. Women should avoid bikinis and tank tops.
