I know they don't charge if you need help changing jewelry or putting in jewelry that has fallen out. I'm not sure about dermals. It wouldn't hurt to call and ask, besides it might possibly have to be a certain piercer. I know it had to be when I got my vch done.
I believe so, if anything it would be a courtesy charge though similar to increasing gauge size.
