You will probably have to have your government issued identification to go to the players club and get a copy of your card. If the players club is closed a pit boss can make you one,(at the table games) if you've lost your ID you sometimes can get them to make a card for you by identifying you by variation of personal information on file but you won't be able to do anything like changing anything on your account or selecting a new pin.
Everyone leaves their card at least once you can get one everytime you go and you points/freeplay/drawing tickets will be on the new one
Just go to the players club with your ID and they'll give you a replacement
