Sorry but it isn't that easy anymore. You have to get new doctors form before any new placards can be issued. Temporary or permanent.

For temp placards: bring your old placard into the DOL and apply for a new placard. They'll need to be renewed in this way every 12 mos. For permanent placards: Renew via renewal card sent from the DOL to your address.
