Lunch is more casual than dinner. I don't feel out off place in jeans (no holes) for lunch.

It a nice family restaurant. I don't think there is a dress code but everyone seems to dress casual.
No. Wouldn't recommend showing up with no shirt or shoes but it's pretty relaxed setting
