Visit DDS Online Services to see if you are eligible to renew and perform other license services online.  Otherwise, please visit the nearest Customer Service Center.  Skip a Step and fill out your required application form before you arrive.  NOTE: You are not allowed to perform a name change or gender change online. You must visit a CSC and provide proof of your name or gender change.
