Try taking you receipt along with tge card and avtivation slip back to the store. They can probably do in depth research on the card. The cashiers name should be on the reciept as well.

I called the visa card 800 number. They told me there was a $50. Cash advance made when I purchased the card. It didn't show on my receipt. My receipt shows I paid $100. For the card. They also told me the advance was done there right after I purchased the card.
