I can't recommend Prevail PT enough! Excellent front staff, straight forward insurance processing and scheduling.The facility itself is clean, welcoming and well equipped. As an older person undergoing rehab after knee replacement surgery, I don't feel intimidated or pressured. My PT provider, Matt, is knowledgeable and professional, helping me at my appointments but also helping to build a home program that takes into account that I don't have a high tech work out set at home!

I played for the Oakland Raiders of the NFL for 7 years. From 1981 until now I’ve had over 60 orthopedic surgeries and procedures. Over the years, I’ve worked with some of the most successful physical therapists on the west coast. However, right here in Shoreline, WA I found the best physical therapist I have worked with in all those years. I am a physically disabled and approaching my 60’s. I have simple goals…. such as being able to stand for more than 15 minutes unassisted, simply walking one or two blocks with my wife holding hands, but most of all being able to fully enjoy traveling with her during our retirement years. Doesn’t sound like much I know... but it sure is taking a lot of hard work. Dr. Algerae Bergstrom is a talented, knowledgeable and compassionate Physical Therapist. She moves me forward with a very detailed plan, in the perfect environment with the right balance of empathy and push to do more. Just the improvements I can see and feel every week keeps me motivated come back. Now my goals are within reach. Dr. Algerae, you and Prevail Physical Therapy are simply outstanding!

I cannot say enough good about Bruk Ballenger and the care I have received at Prevail Physical Therapy. I have had back pain on and off for a number of years and seen quite a few practitioners. Far and away, Bruk is the most knowledgeable and helpful. He actually listens when I am describing symptoms. He does thorough research when prescribing exercises. He watches when I am trying something new and offers thoughtful form corrections when needed so that I can work independently at home. He has shown me stretches and mobility and strength exercises that I had never done before that are key to my recovery. He is patient and I feel "seen." I believe he cares about my wellness and that makes all the difference.

Bruk and his team are extremely thorough and so very patient minded. I have been going here for almost a year, and they have helped me get back to where I want to be athletically one step at a time. Tom has been amazing during my PT process and truly has customized exercises and plans for me to easily follow. They have a personal touch that I have not experienced at other PT providers.

Yes. I am the business owner. For patients not utilizing health insurance as a payment source, we have a discounted cash-pay rate per visit. We also have a further discounted rate for pre-purchasing packages of visits or services, and to answer your question, yes we can work out a payment plan for you.

Specialty services are typically covered by normal health insurance physical therapy benefits if they are an indicated part of a logical treatment plan and meet the definitions of medical necessity.

What to expect on my first appointment?

Before your appointment we will communicate via email or telephone, to discuss; Scheduling Payment/insurance information Demographic intake information Facility access The issues that you need help with and your goals Please arrive ten minutes early to your first appointment to finish up any remaining paperwork not completed beforehand. Your therapy time will be more efficiently utilized during the first visit if all of your initial paperwork is complete before your appointment time. We can also optimize time if you procure any relevant imaging reports or medical notes and send them to us before your first visit, as well write via email, a narrative regarding your current issues, ideally with a history of other injuries and issues, before your first visit. Bring appropriate dress: shorts for lower extremity issues (and appropriate footwear), and tank top/T-shirt for upper extremity issues. Plan on at least 60 minutes for each visit.

What is the cost, and how are payments made?

If we are using your health insurance benefits, then your insurance company will dictate reimbursement based on the evaluation and treatment procedures performed each visit. As a rough estimate, use $125 per visit, so if you have a 10% co-insurance, then your cost will be $12.50. Every insurance plan is different, so please understand your particular benefits. Co-insurance costs will be billed to you after insurance reimbursement. Copays are collected at time of service. Deductibles may increase your up-front cost. Many of our clients opt to pay out-of-pocket for their services. We can supply the necessary documentation for you to provide your insurance company for possible reimbursement if we are not contracted with your insurance company. Our cash payment rate is $130 per hour. Sponsored groups may receive a 25% discount on cash services. Payment plans are available if agreed upon before starting a course of therapy, otherwise payment is collected at the time of service. Cash, check and credit cards are accepted as forms of cash payment. A Ten-visit package can be pre-purchased for $1,000. Health Savings Accounts (HAS) and Flexible Savings Accounts (FSA) may be utilized for physical therapy services, co-pays, co-insurances, deductibles, etc.
