Worst USDA vet I have ever had the misfortune to work with
Tomasz Pa
They are great people with really quick turnaround. I took my dog for a trip to Europe and needed to endorse my dog's health certificate. They did it in just three hours. I used electronic VEHCS (veterinary export health certificate) to speed up the process (no need to ship papers). I wish all US government offices would work that quick and professional.

We had to use the USDA to get our forms for our dogs authorized for import to a rabies free country. They did an outstanding job getting our approved forms back to us in nearly a day, considering the constraints of COVID-19. I'm truly impressed by how efficient the process was. If you're in the same boat as we were, PLEASE make sure you have all the documents you need before you send the paperwork to them.

Staff here are extremely rude, no patience, do not choose this place unless you have no choice, if your staff here let people communicate with them Email only, then I have no idea why there s a tel number on your google information. I just wanna ask something about my dogs health certification, because our local animal clinic told me the health certification will take longer time, but my uncle got cancer and hes sick very bad, I don't think I can wait until they ship my dogs documents back, so I called them for an absolute URGENT, but the lady she didn't even let me finish my problem, she just told me she will send me an email with attachment, but I believe she don't even know what my problem is, and I never got an Email from this department. Also, when she ask me where I will travel, I said China, I have no idea why shes laughing or scorn or something. At least this is a government department, if your staff here cannot work hard, please be nice at least!

Yes. Note: they prefer to do paperwork by courier, you are discouraged from going there in-person.
No...
You have to make appt. I would call and check on delivery date or appt potential
I've only exported out of u.s., but, you have documents that should be filled out and procedures to follow at your origin, start with your vet, they will have procedures that need to be followed and it will depend on point of entry into states, each state will vary on requirements. Once cleared with documents there will be an agricultural entry areas at customs to clear entry.
